All candidates who have
not graduated from an ADA-accredited dental school or program and have been accepted into
a residency, graduate, intern or fellowship program at a North Carolina Dental School or
teaching hospital offering programs in dentistry must:
Submit a completed
application and pay the non-refundable fee of $150.00
Have a letter from the supervising dentist
indicating the location of practice and name of the dentist
Submit official transcripts from their dental
school.
Sucessfully complete written examinations
(sterilization/infection control and jurisprudence)
Submit written
confirmation that he/she has qualified for and is currently enrolled in a graduate,
intern, fellowship, or residency program in the North Carolina Dental School or teaching
hospital offering programs in dentistry
Submit
written confirmation that an ad hoc committee (consisting of three associate or full
professors, only one of whom represents the department in question) has evaluated
his/her's didactic and clinical performance with the point of observation being not less
than three months from the start of the program, and has verified that the applicant is
functioning at a professional standard consistent with a dental graduate from an
ADA-accredited dental school
Successfully
complete a simulated clinical examination:
Submit written
confirmation that the he/she has successfully completed a program of study at the training
facility in:
(A)clinical pharmacology;
(B)
Prescription writing in compliance with Federal and State laws;
and
(C)relevant laws and administrative procedures pertaining to
the DEA;
Submit
a written statement of the total time required to complete the graduate, intern,
fellowship, or residency program, and the date that the he/she is scheduled to complete
said program;
Successfully
complete written examination(s) administered by the Board
