intern permits and information
Intern Application and Information Please note that once your application is received by the Board office, the process may take up to 90 days. Candidate must be a graduate of an ADA-accredited dental school unless accepted into a residency program or fellowship at a Board-approved tranining facility. Candidate must have a letter from the supervising dentist indicating the location of practice and name of the dentist or from their residency program director. Candidate must submit official final transcripts from their dental school, bearing the degree and date conferred. Candidate must sucessfully complete written examinations (sterilization/infection control and jurisprudence) The non-refundable fee is $150.00. The application can be completed via the Licensure Gateway. Once logged in, you may access and complete the Intern permit application. To renew a current Intern permit, you must first create an account in our Gateway System. Once logged in, you may access and complete the renewal application up to 60 days prior to the expiration date. If a permit expires, it may no longer be renewed.
