The Board now offers the following services for obtaining duplicate dental and dental hygiene renewal certificates: Duplicate Requests For Dentists: For Hygienists: You must login with your license number and PIN. Your PIN is the last 4 digits of your social security number. You must have the ability to print from your computer or device. Dentists may order duplicate original licenses. Print the Duplicate Request form found under the Forms link and mail in to the office with a check or money order for the fees. Please allow at least 2 weeks for processing before calling the office to inquire. Hygienists are no longer required to display original licenses at each location so duplicate original licenses for dental hygiene are not provided. Online Renewal for PAs or PLLCs We now offer online renewal for established PAs and PLLCs. You may not renew until you receive notification from our office. Once you do, the notice will contain your corporation number and PIN. To renew your Professional Association or Professional Corporation, click here. To renew your Professional Limited Liability Company, click here. Once you renew and pay the fee, you may print your renewal certificate. |
